If unable to download/print the forms, please arrive 15 min prior to your appointment to fill out paperwork in the clinic

Prepare for your first appointment

  • Bring the following items 
    • Intake forms (if possible)
    • ID 
    • Medical Insurance Card 
    • Copy of your physical therapy referral (if possible)
    • Other relevant medical reports (X-ray or MRI reports etc.)
  • Wear comfortable clothing that will allow the therapist to access your injury site 
    • Therapists often prefer to see the injured area in order to properly evaluate your condition
    • If modesty is preferred, please inform your therapist
